Why are my T17 jaws are not aligned correctly on Titan 4 110?

The below images explain how to tighten the T17 jaw connection on a Titan 4 110 to ensure they are aligned correctly.

Using grips or pliers hold the square piece so that 
its faces are oriented the same direction as the
jaws.

Using a 6mm Allen key tighten the screw in the
center while keeping square piece aligned.
